Scar diminishing gel is a topical, film-forming, semi-occlusive polymer gel applied to intact, re-epithelialized skin for the prevention and conservative management of pathological cutaneous scars—principally hypertrophic scars and keloids—following surgery, trauma, or burns. In clinical practice it is the gel analogue of silicone sheets, chosen for irregular or mobile surfaces (face, joints, small contours). The dominant class is medical-grade silicone systems based on polydimethylsiloxane (dimethicone) in combination with silicone elastomers and resins (e.g., dimethicone/vinyl-dimethicone crosspolymers, MQ-type resins) plus thickeners such as fumed silica to create a thixotropic gel that spreads thinly and cures into a continuous film. Many formulations include volatile cyclic silicones (e.g., cyclopentasiloxane) to improve spread and then evaporate, leaving a dry, flexible layer. Some gels add functional adjuncts—UV filters (for photoprotection of new scars), emollients (panthenol, tocopherol), or inert pigments for cosmetic blending—without altering the primary silicone mechanism. A secondary, smaller category comprises non-silicone pharmacologic gels labeled for scars (e.g., heparinoid/allantoin/onion-extract combinations). These actives are drug products where registered, but they are distinct from silicone devices and are not interchangeable in composition. Silicone gels reduce excessive scarring through controlled occlusion and hydration of the stratum corneum. The cured silicone film lowers transepidermal water loss, normalizes hydration-dependent signaling between keratinocytes and dermal fibroblasts, and down-modulates pro-fibrotic pathways (notably TGF-β–driven collagen overproduction). The barrier also reduces pruritus and erythema by protecting terminal nerve endings, minimizing friction and shear, and stabilizing local microclimate over the scar.
